Bicaval valve implantation

 A 65-year-old woman with an enlarged heart successfully treated by Cardiologists at Rajagiri Hospital. Mary Joy a native of Kolenchery had the first symptoms of this rare heart condition 3 years ago when she had fallen unconscious at her home. She was taken to a private hospital in Ernakulam and was evaluated, she was also referred for a cardiologist opinion and was on medication from then. Until 2021 September she was on medication but her condition started to deteriorate and they started searching options for her treatment. Her husband through a friend came to Rajagiri Hospital for expert opinion. Upon investigations it was found that she was suffering from Tricuspid valve regurgitation. In this condition, your tricuspid valve is in the wrong position and the valve's flaps (leaflets) are malformed. As a result the valve does not work properly. Large mount of Blood leaks back through the valve, making your heart work less efficiently. 

The tricuspid valve normally sits between the two right heart chambers (right atrium and right ventricle). Mary Joy’s tricuspid valve sits lower than normal in the right ventricle. In Mary’s case Valve had severe leak & due to this her heart was enlarging and heart function was decreasing. Her option was to go for surgery in the form of Valve repair or replacement, but it was a risky option considering her age and comorbidities. Dr. Suresh Davis along with other team discussed with patient and her relatives about percutaneous bicaval valve implantation, wherein with the help of a catheter, valves are implanted in two major vessels leading to right side of the heart (superior and inferior vena cava) through vein in the thigh, thereby preventing backward flow of blood from the heart. TricValve Transcatheter Bicaval Valves is a system of two self-expanding biological valves for the treatment of patients whose Tricuspid valve doesn't properly close and results in caval reflux. The prostheses are implanted percutaneously into the superior and inferior vena cava without disturbing the native tricuspid valve. This procedure is rarely attempted in our country, this is the second such case done in our state, the first case was also done in Rajagiri Hospital for a 73 year old Male patient.  The procedure took about one and half hours to complete and the patient got discharged after 16 days with great relief in her symptoms The procedure was led by Cardiologists Dr. Suresh Davis, Dr. Ramdas Nayak, Dr. Antony Pathadan, Dr. Jacob George, Anesthetists Dr. Mary Smitha and Dr. Dipin.
