Patient & Family Rights and Responsibilities

Patient & Family Rights:

1.Right to access his/her clinical records
2.Right to receive care that respects his/her dignity, values and beliefs.
3.Right not to be discriminated based on gender, caste, religion, ethnicity, illness condition, disability, or social background
4.Right to be protected against physical abuse/neglect.
5.Right to be respected for personal dignity and privacy during examination, procedures, treatments, clinical interviews and transport.
6.Right to treat patient information as confidential.
7.Right to participate in decisions about care including acceptance and refusal of treatment.
8.Right to seek second opinion.
9.Right to give informed consent before treatment or procedure (Before transfusion of blood and blood products, anaesthesia, surgery, initiation of any research protocol and any other invasive/high risk procedures/treatment etc).
10.Right to complain and information on how to voice a complaint.
11.Right to receive prompt emergency screening and necessary stabilizing care, as per the hospital’s capacity and facilities.
12.Right to be informed about the proposed care, expected results & possible complications of the planned care and treatment.
13.Right to information including detailed bill and expected cost of the treatment.
14.Right to personal safety and security.
15.Right to identify whom he/she wishes to participate in care & what information regarding their care will be provided to self & family.
16.Right to know the identity of individual providing service and information about the care plan, progress and healthcare needs.
17.Right to know the hospital’s rules and regulations.

 

Patient & Family Responsibilities:


1.Abide by all hospital rules and regulations.
2.Ask questions to the doctor and clarify doubts regarding diagnosis or treatment and participate in decision making regarding treatment and care.
3.Be punctual for appointments. To inform about cancellation or rescheduling of appointment as far in advance as possible.
4.Careful handling of hospital furniture and equipment
5.Comply with the no smoking policy within the hospital premises
6.Comply with the visitors’ policy and be considerate of the patients’ well-being and safety.
7.Follow prescribed treatment plan and carefully comply with the instructions given.
8.Maintain good habits and routines that contribute to good health
9.Not to give medication prescribed for him/her, to others.
10.Not to take any medications without the knowledge of doctors’/ healthcare professionals.
11.Provide complete and accurate information about his/her health including present condition, past illness, hospitalisations, medications and any other matter that pertain to his/her health.
12.Provide complete and accurate information including full name, address and other information.
13.Provide complete and accurate information for insurance claims and work with the hospital and billing offices to make payment arrangements.
14.To attend follow-up appointment as requested.
15.To pay for service billed for in a timely manner as per hospital policies. 
16.Treat hospital staff, other patients and visitors with respect and courtesy.
